Are these vertical or horizontal? It shows both in the picture

The ones I received were horizontal.

Would there be enough room in here to hold both the namecard and another folded piece of paper (placed behind name card)?

Yes, they are somewhat flexible. We often put two badge cards in so if/when the holder flips around (we use a neck strap to hold them) you can always see the name. And the ladies often out business cards in the holder behind their name card.

How can you attach these to a shirt or blouse?

I used lanyards but you can also purchase Metal Badge clips like this to attach to a shirt. http://www.amazon.com/Metal-Badge-Clips-Clear-Straps/dp/B0038NHBBM/ref=sr_1_1?ie=UTF8&qid=1406857093&sr=8-1&keywords=badge+clip

So if I want the vertical badge pictured bottom right, what stock number do I use to order those?

Click on Court House Supplies. Phone number is listed to call them. Good luck
